Learning Disability Services - Liaison Nurse Services

The Lincolnshire Partnership Foundation Trust Learning Disability service provides a range of specialist health services for adults (aged 18 plus) with learning disabilities.
The Learning disability liaison nursing service provides services both within the community and Lincolnshire’s acute hospitals
It aims to:
- Support Service Users with additional long term conditions to access universal services and care pathways, supporting reasonable adjustments where required.
- Work with health colleagues in acute hospital settings to develop admission and discharge pathways to improve quality and access for individuals with a learning disability, supporting reasonable adjustments where required.
- Provide knowledge and skills training to colleagues in Lincolnshire within acute health care, universal community health and primary care services, to raise awareness and understanding of the needs of Service Users.
- Health Action Planning as facilitated via the GP’s annual health check

Referrals can be made to the service by contacting Long Leys court on 01522 577404 and asking to make a referral to Health Liaison Nurses (for people with a learning disability accessing community services) or Acute Liaison Nurses (for people with a learning disability receiving treatment at Lincoln County Hospital, Grantham Hospital or Pilgrim Hospital.
