Children’s Community Nursing
Community children’s nursing (CCN) teams provide specialist support, education and direct nursing care to children and young people with acute health care needs, long term conditions and those requiring complex or palliative care within their own home. Care focuses on preventing hospital admission whenever appropriate and facilitating early discharge from hospital.
The Teams provide care coordination and promote integrated working with other professionals involved in the child’s care whilst working in partnership with the child / young person and their family / carers.
The CCN teams provide direct care, advice and support to all children and young people 0-19years (as appropriate) with an identified / specific nursing need who are living within the Lincolnshire geographical boundaries.
Co-ordination and provision of holistic individualised packages of high quality evidenced based care which addresses the child’s care complexities.
Provision of direct nursing care / intervention for example: removal of sutures and staples and application of dressings following trauma, burns / scalds, pin site care following limb lengthening, administration of intravenous antibiotic and other drug therapy, weekly line flushing of central lines and ports, blood taking and administration of chemotherapy, passing nasogastric tubes and changing gastrostomy buttons.
Teaching the child (where possible), parents and carers to carry out specific nursing care, including highly skilled technical nursing interventions and invasive procedures if required. The CCN ensures the child and family feel confident and are competent through on-going assessment of their practice.
Provision of opportunistic health education for the whole family, incorporating health-promoting strategies within the child’s care plan as appropriate ensuring that the family/carers have a suitable level of information, preparation and training that enables them to care for their child in the community.

The teams cover the whole of Lincolnshire within their own geographical boundary.

Referrals are accepted from healthcare professionals.
Children living beyond the geographical boundary who are registered with a Lincolnshire GP or are under the care of a ULHT Consultant will also be eligible for the service.
NB: Most young people 16-19years on the CCN caseloads have special needs and are under the care of a Consultant Community Paediatrician.
Referrals may include:
Children with either an acute nursing need, a long term condition or complex health need. This includes those children with a life-limiting and / or life threatening condition and children requiring palliative and end of life care.
