Local Offer

Description

The Circles of support work uses person centred tools and is based on a person centred way of thinking. We can therefore assist with a person centred plan for each person who would like a circle of support.  If the circles of support is in a educational establishment each person will create their own plan; which will support with the 'Moving On' phase at school or college

There is an expectation if somebody has a specialist need that they provide the access or equipment needed. This is not possible within the funding of the service; however as much support as possible will be given.

The service has worked with a wide range of conditions and disabilities, such as younger people with autism who wish to stay in main stream school, adults who live independently in the community with support and people who require 24 hour support. We have also assisted people who are in transitional phases of their life, such as changing service provider, or leaving college and in preparation for adulthood.
