4All Children’s Disability Register for children and young people aged 0-25 with a disability and/or additional needs in Lincolnshire.
Registering your child on Lincolnshire’s Children’s Disability Register helps us identify and plan for the needs of children and young people with disabilities, their parents and carers.
The register also operates as a contact list to provide information and consultation about short break opportunities or any special educational needs policy changes.
Contact the Families Information Service on 0800 195 1635 if you’re not sure if your child is eligible for the register or if you need help with:
- queries relating to disability/additional needs
- other family issues ranging from childcare to benefits, leisure to support groups
Registration is voluntary and is not linked to or a requirement of receiving social care services, which are assessed separately.

4all is available to families with children and young people aged 0-25 who have additional needs and/or disabilities. You can join 4all by completing an initial referral form, after which will contact you to take some further details.
You can contact us directly to join 4all, or a professional can complete the form on your behalf.
