Circles meet as often as the focus person requires, helping to develop and implement a person centred plan. The person centred plan can be used to inform the Education Health and Care or transition planning. 


What does a person centred plan do?

  • Helps to identify where the person is now and what they want for the future
  • Help to identify what support is needed and who can help them achieve their goals

Circles can also help you to complete a 'one page profile' which is useful to take to a new school/college or employer, to let them know important things. A one page profile can include information including what is important to you, how you can be supported and your communication needs.


How much does it cost?

Circles is free of charge, and is for people of any age.
